we didn't use canned mushroom soup in this recipe. Instead, we used onion soup mix to bring out the flavor of the pork chops and create a delicious gravy. If you want to make it entirely from scratch, you can make your own onion soup mix (we have that recipe as well).
A slow cooker does an excellent job of keeping pork really moist and tender. The steam that is created in the slow cooker helps keep it from drying out. If it does get dry, it is because the pork was over cooked. If that happens just continue to cook it longer until it falls apart and shreds. Then mix it with the gravy. Dinner done.
